Learning Objectives
Identify the D-SNP behavioral health (BH) teams functions, services and treatments that will benefit the members
Distinguish the difference and significance between a D-SNP BH referral and a BH consult when requesting assistance from the BH department for the members
Recognize the common D-SNP behavioral health conditions which would benefit from a BH consult
Prioritize the ongoing collaboration between the D-SNP BH team and the case management (CM) team, with regards to members who have BH needs, or potentially both BH and medical needs. Co-presenting BH members to the D-SNP's Interdisciplinary Care Team (ICT)
