Privately Paid Case Managers/Health Advocates: A New Practice

Learning Objectives
The participant will possess a comprehensive understanding of the forces driving the need for private case management and how that need manifests itself with families, trusted advisors, clinicians and attorneys.
The participant will describe techniques for successfully building collaborative relationships with institutional as well as community providers, such as discharge planners, insurance company representatives, home health agencies, elder and estate attorneys and trust officers.
The participant will be conversant with outcome measures that show value to clients and other constituencies using clinical, financial, and quality of life measurement tools.
