Proactively identify patients who will need post-acute care and communicate regularly with patients, families and caregivers about post-acute care plans and the patients progress.
Collaborate with interdisciplinary teams to develop a comprehensive post-acute care plan that addresses my patients individual needs.
Coordinate with post-acute care- providers to ensure a smooth transition of care and continuity of care.
Monitor my patients outcomes to assess the effectiveness of the post-acute care plan and adjust the plan as necessary to ensure quality care.
