Identify best practice strategies ACO case managers are discovering and using to drive change in managing high-risk ACO populations to improve quality, efficient care and outcomes including strategies to address social determinants of health, health equity, and workforce development.
Develop policy and advocacy recommendations that can help shape future ACO policy to best support ACO case managers and the ACO populations served.
