Care Transitions

Learning Objectives
Understand which services are offered at each post-acute level of care including rehab facilities, long term care (nursing homes), assisted living facilities, home care programs, and day care programs.
Verbalize understanding of what considerations go into securing and then transitioning a client / patient to another level of care.
Identify the steps clinicians must take to facilitate and secure a safe and timely discharge for a client / patient so their individual needs and care goals are met.
Understand the issues and implications when care transitions dont go well and the affect on the client, family, and both providers.
