The learner will verbalize the steps hospital or rehab discharge planners take to facilitate a safe, secure, and timely discharge to see that a individual's needs and care goals are met.
The learner will describe how to advocate if they believe the discharge plan is not safe or doesn't address care goals.
The learner will state which partners are available in the care process, including transitional and permanent partners. .
