Differentiate between normal aging and dementia so that case managers can recognize the difference in their patients, and advise them on whether they need referral to a doctor for further evaluation, to improve outcomes.
Understand common causes of dementia, so that case managers can better understand the diseases their patients have, which can lead to better recognition of the resources patients need, to improve access to care and outcomes.
Understand strategies for protecting brain health in people with and without dementia, to improve case managers' ability to to advise their patients on best practices to slow dementia progression, to improve outcomes.
