Learners will understand the IMPACT Act and the CMS proposed changes to discharge planning that will result in changes for hospitals.
Learners will be able to identify the CMS discharge planning standards that all hospitals must follow.
Learners will understand when the discharge summary and instructions will have to be provided to the primary care physician under the proposed rules.
