ABOUT THE WEBINAR
Social factors play a profound role in an individual’s physical, emotional and financial health. Poverty is among those conditions caused by a person’s particular social determinants. As with any other medical or mental condition, the treatment for poverty as an environmental medical condition requires intervention based on science and access to viable, reliable support systems, as well as ensuring that individuals have a clear understanding of what recovery looks like in their lives. Case managers are the connectors for clients whose health depends on ensuring these essential environmental needs are met. In this webinar, the speaker will share evidence-based strategies for improving outcomes for those who suffer from poverty. 
 
After the webinar, participants will be able to:

  • Describe evidence-based standards of care for the treatment of poverty as an environmental medical condition 
  • Identify ways to leverage standards of care and best practices to better address the social determinants of health that are contributing to poverty 
  • Evaluate current community partnerships and identify ways to expand these key relationships and grow new partnerships to support the treatment of poverty
