Case managers are the connectors for clients whose health depends on ensuring these all-too-human needs are met. In this webinar, the speaker will share strategies for establishing connections to community resources and developing relationships with these important partners.

Featured Speaker: Jessicca Moore, MSN, FNP, Director of Innovation at Petaluma Health Centers
Jessicca Moore, MSN, FNP is the director of innovation at Petaluma Health Center. She earned a master's degree in nursing from UCLA and a certificate in rural and community clinic management from Sonoma State University. Moore has been with PHC since 2005 and helped guide the organization through its transition to team-based care and patient-centered medical home status. She approaches problems through a human-centered design lens, leveraging her experience as a clinician and a practice leader to her work in quality improvement and innovation to improve outcomes in vulnerable populations.
