In a perfect world, providers could help people get healthy and stay healthy by addressing all the conditions that affect overall health. It’s called an “upstream” approach—one that takes into account the social and environmental sources of health problems that make their impact where we work, live and play. This session will outline the tools and considerations case managers and health care organizations can use to implement an upstreamist mindset.
