Social determinants of health: Connecting to community resources

Social factors such as food and housing security, transportation and personal safety contribute to poor health outcomes. Case managers are the connectors for clients whose health depends on ensuring these all-too-human needs are met. In this webinar, the speaker will share strategies for establishing connections to community resources and developing relationships with these important partners.
 
After the webinar, participants will be able to:

  • Describe effective strategies for implementing social determinants of health screening in primary care. 
  • Identify ways to leverage technology and data to better address patient's identified social determinants of health.
  • Evaluate current community partnerships and identify ways to expand these key relationships and grow new partnerships. 

 


 

Featured Speaker: Jessicca Moore, MSN, FNP, Director of Innovation at Petaluma Health Centers

Jessicca Moore, MSN, FNP is the director of innovation at Petaluma Health Center. She earned a master's degree in nursing from UCLA and a certificate in rural and community clinic management from Sonoma State University. Moore has been with PHC since 2005 and helped guide the organization through its transition to team-based care and patient-centered medical home status. She approaches problems through a human-centered design lens, leveraging her experience as a clinician and a practice leader to her work in quality improvement and innovation to improve outcomes in vulnerable populations.
